Application Forms for Cornerstone Lodge

Our goal is to make joining our recovery community simple and supportive. Start by completing an application form online or in person. Once submitted, our team will review your information and contact you to schedule a brief interview to discuss your goals, needs, and readiness for our program. After the interview, you’ll be notified of your acceptance status—usually within 3–5 business days. If there’s a waitlist, we’ll keep in touch and provide updates until a space becomes available. Once approved, we’ll guide you through move-in orientation to help you feel welcomed and prepared to begin your recovery journey.

On the right-hand side of the screen, you will find the necessary forms. Please download and complete them at your convenience. Once finished, return all applications to admissions@albrecovery.org. You will be contacted within three business days to schedule either a phone or in-person interview.

HIPAA & Confidentiality

At our recovery home, your privacy is our priority. We follow all HIPAA (Health Insurance Portability and Accountability Act) regulations to protect your personal and health information. Anything you share with our staff—whether during intake, counseling, or daily support—is kept strictly confidential and used only to provide you with the best care possible.

42 CFR Part 2

We also comply with 42 CFR Part 2, a federal law that provides extra protection for individuals receiving treatment for substance use disorders. This law prevents us from sharing any information about your participation in our program or your treatment without your written consent, except in very specific legal situations.

Release of Information (ROI)

If you’re involved with a community justice program—such as probation, parole, or treatment court—you’ll be asked to sign a Release of Information (ROI) form. This allows us to share limited, necessary updates (like attendance or progress) with your supervising agency while still protecting your privacy. We only share information that you authorize, and you may revoke consent at any time.
